$99 enrollment fee- waived if patient enrolls at comprehensive new patient appointment or before June 1st 2021.
With small monthly payments, usually lower than the monthly cost of dental insurance policies, you and your family can be covered!
The price per month depends on level of periodontal treatment (dental cleaning) is appropriate for you. $37- $67 per month.
*Two no-call/no-show appointments will void membership plan
